   Call COREhealth (253.857.8000) to schedule your first appointment. Then fill out and send us the New Patient Questionnaire. This will confirm your intention to become a new patient.

WE HAVE TWO KINDS OF NEW PATIENT APPOINTMENTS.

     1.  Nutritional, Homeopathic, or Cranial Osteopathic
              This means you do not want to take hormones.
     2.  HBT (Hormone Balancing Therapy)
              This means you want bioidentical hormones.
 

CHOOSE EITHER #1 OR #2 BELOW, (If you want a combination or modification of the two kinds of appointments, please call)
 

#1) PATIENTS WHO DO NOT WANT BIOIDENTICAL HORMONE BALANCING THERAPY (HBT):

Send to us:

• Print out and complete the New Patient Questionnaire. Mail or fax it to us no later than one week before your initial appointment.

Fill out, then bring to your appointment:

• The Symptom Survey

• The last page only of the Dominant Nature Questionnaire

Read and bring to your appointment:

#2)  PATIENTS REQUESTING HORMONE BALANCING THERAPY (HBT):

Send to us:

• Print out and complete the New Patient Questionnaire. Mail or fax it to us no later than one week before your initial appointment.
   

Fill out and bring to your appointment:

• The Symptom Survey

• The last page only of the Dominant Nature Questionnaire

• The completed Supplemental New Patient Questionnaire

• The signed HBT Consent Form

• The signed HBT Benefits and Risks form

• The signed High Risk Disclaimer
